could you remove patchh-channels::chan_sip.c from /usr/ports/net/asterisk/files and try rebuilding from ports ?

that's the only patch which seems to touch chan_sip.c, and it does seem to add some loose source routing functionality. that could be causing it to crash (in some yet to be explained way) when you're doing a SIP forward.
