I think it is clearly a configuration issue. What version of asterisk/asterisk-gui are you using?
The asterisk GUI makes two extensions for bandwidth.com extensions (to match the plus). Might be nice for redundant PRI uplinks where the telco will switch over all your DID's to your secondary circuit in the case that a primary circuit fails. Otherwise this would require duplicate code on each trunk. Or maybe making a multiselect option for that where multiple trunks have the same DID list. Just tossing out ideas.
