I had this problem two weeks ago, and it seems to be gone as of yesterday. However, I did end up changing two things at once: I used a newer CVS version, and I changed the DTMF mode in sip.conf. As far as I know either one might have solved the problem, so if CVS doesn't help, try changing the DTMF mode as well.
