Just a quick note, make sure /var/spool/asterisk-fax/ exists and is writable by whatever asterisk runs as. Assuming your connection is good enough, I had "LINE ERROR"s because app_rxfax aborted right after the handshake as it would not write the output file. I think there is a debug mode in app_rxfax that may shed some light. Otherwise see Alexander's reply about the connection quality.
