Issue resolved, Thanks Digium!
The module slot closest to the bracket that contains the connector is the last slot. I assumed that the first slot would be at the bracket.
Silly user error, never assume.

I do not think so by reading the documentation however I have changed the settings and still get the same error when starting Asterisk

Hi,
I believe that you have inverted fxo_ks and fxs_ks into your zapata.cong file "signaling=" declaration...
Invert and redo the tests.
