There are plenty of small form factor boards you could start with
(http://www.linuxdevices.com/), but I assume you'll want something with
one or more analog telephone ports.
I would also be interested if anyone knows of any hardware that's small,
affordable, can run linux, and can have FXO/FXS ports.

I'd recommend the VIA EPIA PD series of boards. If you want to be safe
load-wise get the PD 10000 which has a C3-2 (Nehamiah core) processor at
1Ghz. The board has two ethernet ports and one PCI slot. Combine this
with Astlinux and you have a nice router and PBX that can handle a small
office/home office needs quite well.
The Soekris 4801 is another choice, but you will need to provide power
to the PCI card to handle the FXS ports, plus the digium TDM cards don't
fit in the standard case.
