inside, the outside never hears one, what does this mean?
Why don't you record the call at asterisk? Leave the zaptel settings as default, i.e. standard echo cancel and rxgain=txgain=0. Don't use MixMonitor, just leave everything as in and out. It will help you isolate whether it's PSTN echo. If you don't hear any echo in the recording, then the problem is most likely your phone.
