I have seen this with Polycoms, ZIP2s and occassionally with Linksys 941s, but only intermittently. Sometimes a powercycle will clear it and sometimes not. We've never figured out what's going on, but we think it is something to do with NAT and the phones not exactly sticking to the spec, but that's only a presumption.
