Gsm is what I've been using. The problem could be a "duplication" problem - if there are 2 or 3 foo.* in the v/l/a/s dir, Asterisk "flakes" trying to process them all. I've had that happen with a foo.wav and foo.gsm in the same dir. Also check permissions.
