0) Use a more descriptive subject.

1) Set "auth=plaintext" (only for the duration of the debug session) and 
enable iax2 debugging on the CLI.

You should see the "NEW" request and the passwords in plaintext.

Verify the username, password, context, and extension all exist.

2) Reply with the sanitized console output.
