Have to say, the new DIAX is heaps better! Calls seem to be going through a lot cleaner (not breaking up as much) and is almost the same as Firefly in terms of this.
IAXcomm is still unusable in this regard. Error's from DIAX: (Running Win98SE with all the latest patches on a machine windows was installed on 3 days ago - msvbm60 was missing and I downloaded it from Micro$oft on the link you provided) During a call it crashed, but did not kill the call...as long as I didn't click on OK it kept going. Obviously I couldn't access the gui though while it had crashed... DIAX caused an invalid page fault in module MSVBVM60.DLL at 016f:6601455a.
