I think what he means is this: I can have extension 104 defined in multiple contexts, for instance if I host virtual pbxs for multiple customers on one * box. The syntax of my * conf files requires <exten>@<context> if I want to differentiate between these extensions. If you're using the * box for one business, and you ensure that the same extension is not used in multiple contexts you're ok. But otherwise...
Same goes for voicemail -- I can have voicemail box 104 in the voicemail context [great_customer], and another 104 in voicemail context [really_great_customer].
