This is incidently a know issue with cisco gateways.  They can only
configure vad per box rather than per call.  And given that most
customers of sip providers willb e using sip equipment that supports
it, and most of thost customers will want to bandwidth savings it is
as sure a bet as anything that sip providers using cisco will always
have vad turned on.

Asterisk just has to cope in these cases.

To the OP:  grep for silence in chan_sip.c; try commenting that line
out and see whether they like that better....
