Short answer: try new FXO modules or a new card.

I've struggled with this for about a month. I've returned one TDM400 card, got a new one. Had same problems, Digium support installed a patch for zaptel, no difference. Then I diagnosed one FXO was dead. Got a replacement for that FXO. Suprisingly the new FXO module that arrived from Digium did not have this "loud noise" issue for that line. While I was having occasional "loud noise" on all the others. Migrated to another PC server, no difference.

Then I got four new FXO from Digium (btw, no S/N on them!!), they claimed "this will fix all your problems with the card". Got same problem again on just one FXO line. Replaced that FXO with FXO they've sent me as the replacement the first time. And voila. System works in production for more than 2 weeks now, no loud noise so far. (I pray so it stays like that forever).

If anyone can comment what's the schematic difference between TMD400P Revision E/F and Revision H. As well as how new FXO modules are different from the old ones, that would be great.
