Are you running asterisk as user asterisk ?
If so, you need to add this user to the dialout group.
Otherwise it won't have access to the modem.

I'm running asterisk with user 'root'. Asterisk user is in the dialout
group and I try to start asterisk as user asterisk, with the same result.
