Over the last month I've been playing with the Realtime configuration. For
the most part you can do anything within the mySQL database and at most you
just have to reload the .so files if you make changes. I just wrote a quick
PHP app so that I can quickly edit the tables from the web. It could easily
be edited to connect to the manger interface to do those reloads from the
web interface. Unfortunately the documentation isn't great so sometimes it
needs to be debuged with a fun tail -f /var/log/asterisk/full

The main goal of this was to be able to have users edit their own setups
without any interaction from me. IE: get DIDs, wholesalers point them at
their own servers, end users point them at their own endpoints, etc, etc.

Overall, I'm happy with the move from flat files though. It's extremely
easy to mirror mySQL databases and you can easily keep a hot spare that's up
to the last query.
