There are many things we take for granted, and this is probably one of those "simple" obvious issues that most of us tend to overlook. I've looked at the voicemail config file at least 10 times to see what went wrong & until you pointed it out... it wasn't flashing in front of my eyes. If you look in /usr/src/asterisk/configs (or wherever you store your sources), you'll find all of the sample configs that come with Asterisk. I often look things up in there.
