Poor Sound Quality

I've got a customer who's experiencing bad sound quality on their
system. The last part of a sentence is dropping off. Additionally, to
varying amounts, bits are dropping out in the middle.

I'm using the new TDM2400P card with a quad FXO port card and a hardware
echo cancellation, on asterisk 1.2.5 with Polycom 501 phones using ulaw.

I'm thinking this might be the echo cancellation or it might be the fact
that I have rxgain and txgain set to 8 and 5 respectively to compensate
for low volume.

Any ideas?
