Dave, This may seem obvious, but it's worth ruling out. Have you set the DTMFMode in sip.conf to 'inband'? http://www.voip-info.org/wiki/view/Asterisk+sip+dtmfmode If you only set the handytone then it would dial properly but Asterisk would still be waiting for an out of band signal.
