You will use descending sequential (for outgoing), they should use ascending sequential (for incoming)
-Hunt Group, Virtual Group
You shouldn't need these, unless you have complex traffic requirements. Generally this can be controlled in asterisk, which is a much better way to handle it.
-External Name & Number Display (does this mean I can set my own name
and number on the fly)
You'll probably want to be able to control it on outgoing, although that also means you'll need to handle that in asterisk.
-Digit Substitution required
None. You want them to send DNIS (the number that was dialed; i.e. the DID number).
-Backup D required (y/n)
No. You can only use this if you have more than one T1 providing the PRI, and also, with Astersk, you're better off with multiple D-channel control if you have more than one T1 (it's easier to configure).
-Line coding & framing (I think this is B8zs/esf)
Correct
-ISDN protocol (I think this is NI-2)
Correct. DMS100 is fine too.
-Release line trunking (Allstream says it's a $75 charge)
Probably 'no'. If you are doing a lot of external transfers, this may reduce trunk usage, but then you lose control of the call, so won't be able to do CDR on it. Also, I'm not sure if Asterisk handles this correctly yet.
-Digits to outpulse
DNIS - 10 digits.
