Hi Lloyd,
I haven't experienced that before but I had other problems with CallerID on 
FreePBX for outgoing and it's time consuming to find those given the structure 
of FreePBX. But, if you are forwarding calls to only one number then maybe you 
should try this:
Browse to the extension that receives the call in FreePBX and put the following 
under the Dial field which is under Device Options:
SIP/g0/14165556666OrSIP/sipTrunk/14165556666  (if doesn't work, try 
SIP/14165556666)
Currently the field should look something like:
SIP/200   (I am assuming the extension number to be 200)
This should take care of the Caller ID and Caller Name and pass the call as is. 
Oh, unless you have set or forced another CallerID and CallerName in Trunk's 
settings which you should remove.
