Would it not depend on the initially-called phone-number's carrier? Since
that is where the forwarding is done? But, is there a way to determine what
that originally-called number is? from the callee's perspective? (On my
asterisk machine/DID?) i.e. I receive the call on my asterisk box, forwarded
from another number on carrier X where the caller has dialed to and he gets
forwarded to my asterisk DID. I get his CID, but I would like to know where the
call was forwarded from.

That is the reason I am convinced that it is a BELL problem. In both
cases he had to call Bell from another phone to get them to disable the
forwarding. I say "forwarding" because that is how it behaves, although my
acquaintance insists that he did no forwarding on his phone. During the period
of this behaviour, his phone is continually ringing and if he picks up his
phone he hears my IVR.

thanks for any clues/ideas you may have,
