Replying to my own post...

I now believe this is a bug introduced between 1.4.29 and 1.4.30 and may affect the 1.6 branch as well. I think it is related to this Asterisk bug

How does one go about getting this registered as a bug - can I re-open the tciket, do I open a new ticket.

I am not a programmer so probably couldn't fix this myself.

It does create the file but then it is never sent with the vociemail message.

Any thoughts on debugging further?
