I am making the assumption that at site A you dial NN to access the trunk plus 2XX to reach the extension at site B.
The second assumption is that you are using FreePBX.
You could try creating a new CUSTOM extension at site A such as 1XX.
Enable Follow Me, and for extension 1XX enter NN2XX# in the Follow Me list. (The "#" is necessary)
Check to be sure it works by dialling 1XX from site A to call 2XX at site B.
In your IVR ("Dial 3 for John Smith") for option "3" send it to extension 1XX, which will be forwarded to 2XX at site B. To make send the IVR to a PSTN number, follow the same example, but the Follow Me for extension 1XX would be something like 9X416NXXABCD# where 9X is the code you use to get an outgoing line and 416NXXABCD is the PSTN number where you want to send the IVR call. Again the "#" is necessary. Someone else may have a better solution, but this works for me. I hope this helps.
