I haven't seen any replies to your question, so...
perhaps you can adjust this to your needs:
exten => <called extension>/<calling
extension>,priority,SIPAddHeader(Alert-Info: info=<Bellcore-dr<nn>>)
exten => <called extension>/<calling extension>,n,Dial(...)
I'm doing something like this in vanilla asterisk 1.4. It works well, but
requires a few lines added to your extensions.conf file for each
extension-pair. If you have a large number of extensions, it can get tedious
quickly.
The value of <nn> in "<Bellcore-dr<nn>>" is dependent on the type of the
phoneset at the called extension and the ringtype that you want.
