For what its worth (nothing? :-) we also had similar problems which
turned out to be due to the handset's DTMF tones.  We used (still do)
Snom M3 DECT wireless handsets.  Up until the most recent firmware
update (we are now on v2.02) the Snom phone (or base station, I'm not
sure which) would lose its mind and send the wrong tones to Asterisk.
To my ear, the tones sounded fine, but not to Asterisk, and voicemail
(as well as IVR phone services) were all unusable.

The short term solution was to reboot the base station about once a week.

The long term solution has been to move to V2.02 of the firmware when
it became available, and all has been well since.
