Hi,
After the firewall settings I would check that the connections from WEB are allowed from Asterisk's configuration arguments. I have a question: wich are the permissions of "asterisk.ctl" file? Maybe the connections from WEB are blocked because the file is "not" visible". Try (also) with levels equals to "777" if possible and check that the settings presents into the file "asterisk/manager.conf" allow "Web" connections. (The firewall restrictions maybe another cause).
