Actually, NVFaxDetect() and NVBackgroundDetect() are included in the default 
Asterisk 1.4 distribution.  Though Asterisk 1.8 fax detection should be more 
robust.

I'm not sure if mISDN adds any wrinkles, I sure Michael will comment if it does.
