While it is written to remove the HTML Entities in the callerid, it is very 
simple to change for your purposes.

Note that the variable "${agi_calleridname}" is the channel's callerid name, 
and "${agi_callerid}" is the callerid number.

Simply set "CIDNAME" with the value (name) you find from your lookup.

Accessing this AGI from your extensions.conf dialplan is quite simple... (name 
it what you wish)

exten => s,n,AGI(cnam-fix.agi)
