Give Matt's idea a try, if that does not work before the Set(CALLERID(num)...
try adding: Set(CALLERID(num-pres)=allowed)
Also, the policy of your SIP provider may have changed and may only allow
setting the outbound CALLERID(num) to a number you control with them.
