If there is an active region, I managed to customize
`reftex-index-default-macro'.
However I must say that I find the default behavior of reftex-index
annoying, if one has made up his mind in either generating an index or a
glossary or a nomenclature. So IMHO, at least optionally it should
be possible to deactivate this prompt.
