Hi, I set that variable directly by evaluating the sexp. It seems
there is a caching mechanism that cref doesn't reflect the newly
added cref items, (also not applying the change of this variable
which is correctly setted)

How did you set the variable? If you use the customize
interface (M-x
customize-variable RET
reftex-label-ignored-macros-and-environments
RET) you can set the variable for the current session and save
for
future ones, too.
