I always play with attack sounds turned off now. And I always play with NVDA. Best way to play in my opinion, hands down. The big scary warning in game that tells you how dangerous it is to play with a screenreader...well, it might be true of things like Jaws, but NVDA throws basically no problems and hugely improves the experience for me.
