For PDFs, its impossible to read math directly. Your options there are to use InftyReader (which costs over $1,000) to OCR the math into LaTeX, or preferably, get the LaTeX source directly from the author.

For word files, if you have MathType and MathPlayer installed, you'll be able to read it with NVDA if the equations were prepared using Microsoft's equation editor or MathType.
