I have no idea as to a guide,but its eazy enough.
First,don't update your device over air. I.E directly with the device. use itunes instead.
backup your device on to itunes,just in case. with all those things done,all you need is the ivasi0n tool,run the exe file,connect your device,click on jalebreak your device and caboom. the process will start.
all you need to do than is follow the onscreen prompts on the ivasi0n tool,and you'll be out of the jale in no time at all.
I have no idea about NVDA or other screenreaders but jaws reads the ivasi0n UI farely well,with the jaws curser,of course.
BTW,if you are using IOS6.02 or lower,I will just suggest jalebreaking rather than updateing and jalebreaking.
As for tweeks,the one that I use and find the most useful is the airblue shareing app,which lets you transfer and recieve files using your bluetooth,which is one of the most anoying thing I find about the stock IOS OS. It doesn't wo
rk on IOS7 yet,though there are things you can do to make it work.
