No idea on the save file, I've always just used the one and replaced it no trouble.
As regards mutations, there are a couple of later feats you can get that can let you benefit from your mutated body in combat, but in general the mutations are more aesthetic and gameplay based than anything else and tied more to the game's possible endings.
In the multiplayer version however things are very different. Each mutant body part comes with its own set of powers, which you can then learn as part of your permanent power set, indeed the combat in the multiplayer version sort of resembles a ccg with you having to collect different powers to make an efficient set, though of course other than for rp purposes mutation forms aren't tied to endings etc as much.
