For reading I use QRead which I bought around Christmas last year, works just fine for what I need.
As for converting files  I have for quite a few years now used http://www.robobraille.org/
Supports the filetypes I need and is really easy to use. I even managed to solve a capcha with it once, but that took a hell of a time, so won't recogment that
