I had such problems with win speech recongition addon. I think some addons might create similar problems. Reset nvda to complete defaults with addons removed, and try installing addons one by one. That way you can discover the addon that's causing the problem. Also you don't need an addon for skype. Just use it right out of the box. It's accessible enough.
