This is one big can of worms, and how could it be handled?

Like replay-gain?  When encoding with flac you specify a -premph option
to tell FLAC it has pre emphasis.

When FLAC is decoding much do we say --apply-emphasis ?

In most cases you'd want FLAC to apply the emphasis because the end
result would be audio.  But in some cases you might just want to
transcode the file to another format.

I'm gonna have a play around with iTunes and lossless.  Because if
iTunes is applying the emphasis to the lossless file then it is doing
wrong (as in your audio is now not lossless), and if it is ignoring the
pre-emphasis when doing lossless then Apple have a issue too :D
