I edited Claire's revisions to this chapter and added some pix and
text. It should now be up to date for V3.2 and in the new template.
Chapter needs further checking, especially to verify that instructions
are clear and do work.
Claire, I moved the Form Controls Reference back to its original
location in the chapter, because I don't think it's good practice to
break up a how-to section by including long reference tables. I
realise this is a matter of opinion about best practice in a linear
document like this: whether to link to details or include them in the
sequence.
The revised chapter has been submitted for further review.
