Thanks, Jared. Some weirdness in our files causes some of the x-refs to spontaneously change from the correct ones that are in the chapter files to wrong ones in the book file. So the x-ref goes to where it says, but it's the wrong x-ref to begin with. (Did that make sense?) Anyway, thanks for spotting. Usually these start occurring about half way through each chapter; the ones at the beginning of each chapter are usually well behaved.
