Many thanks to Joe Sellman for asking a lot of difficult questions in his review of this chapter. Joe, I didn't follow all of your suggested solutions, but they did make me think seriously about the structure and content of the chapter (which had not been critically examined for far too long), and I have made a lot of changes: reorganisation and new & changed content (and some dropped). During this process, I discovered some errors in the Getting Started guide and fixed them as well.
