I've accepted most of your edits (I rejected only a few minor
things). I've put in Martin's edits and a few more of my own; you
can see them when you show changes. I also added a comment to the
note you left in the Bibliographies section.
I've uploaded the amended file but I have not submitted it, so it
isn't on the review list. This time I think it really is ready
for final checking and publishing. :-)
