... I have a suspicion as to what might have happened the second time I tried it, now that you mention this as a possible problem. When I first did it incorrectly, calling it a "document", something did get uploaded with that file name. (But not uploaded correctly.) Then when I tried it again calling it a "file", it may have failed because there was already something there with the same name.
That's very likely the problem. I have now deleted the blank files, so you can try again with a completely new file. Should work okay, I hope.
