ISP mode is disabled while in debugWIRE (I suspect this is due to RESET pin 
being reallocated).

The way I have used to get out of dW is to use HVPP or HVSP mode (depending on 
the device).  That's what my shield does.  You can also use the Dragon for 
this, but I don't think it can be done with avrdude (unless there is a way 
avrdude can invoke HVPP/HVSP mode).  I have used AVR Studio for this in the 
past.
