Philip, Well, if you have setup your coverage group to handle action messages, you could experience this.
This will happen if - after the coverage plan has been run - you have planned orders for a given item, while at the same time have an existing purchase order for the same item, that Axapta will suggest to increase in qty. Let's say that you have a purchase order of 100 pcs, while you have a planned purchase order of 50 pcs. If setup properly, Axapta will suggest that you cancel the planned purchase and increase the the current purchase order by 50, so that you end up with 150 pcs. You will then see an action to increase the current PO, while cancelling the planned order. Just remember, that you will also have to consider positive and negative days.
