Hyperdoc now gets all of its page and patch file information directly from bookvol7.1.pamphlet. The pages subdirectory is gone. Since htadd knows how to process pamphlet files directly we no longer need separate files.
This reduces the number of files by a couple hundred as well as the time to process the files. The htadd function is much quicker. In fact, it is so fast that it appears we no longer need to build the ht.db file at all. However, removing this file will be the subject of a later update.
