Bad default namespace on response

That still doesn't help.  When using DOM, adding namespace declarations to an
element (with or without prefix mappings) must be done a certain way to be
picked up properly.

I just ran a couple tests on this and came up with zero problems.  I was able to
reproduce your problem by improperly declaring the namespace on the DOM.
