Web administration frontend displays wrong information on phases

The web administration frontend has a page called "Available phases"

- this page lists "OperationInPhase" twice: under "System pre-defined phases" 
and under "user defined phases". According to all other documentation, this 
phase should be a user-defined phase.

- the page also lists "PreDispatch" and "Dispatch" as user-defined phases for 
InFaultFlow. As far as I understand it, these two should be system-defined 
instead.

- in the phases lists for OutFlow and OutFaultFlow the phases "Policy 
determination" and "MessageOut" are missing, while these are listed nuder 
"Global Chains"

Some documentation mentions a phase called "PostDispatch". If this is still 
supposed to exist, it is missing in the list of phases and also in the 
configuration file axis2.xml

In general, the whole phase concept is very confusing in its current state, as 
the pages "Available phases", "Global chains" and "Operation-specific Phases" 
do not correspond to each other and do not correspond to the available 
documentation either.
