Improve Fault handling

Fault handling was not working properly on the server side when an endpoint
throws something other than a custom exception. We now handle that case.
Also, the framework is in place to resolve fault names to actual exception
classes if an endpoint method happens to declare several custom exceptions that
it may throw.
